How long it lasts

Can be persistent and is one of the longest-lasting GI side effects. Managing it proactively is more effective than reacting to it.

What helps

  • Increase fluid intake significantly — aim for at least 8–10 glasses of water daily
  • Add soluble fiber gradually (oatmeal, chia seeds, flaxseed, psyllium)
  • Daily light movement — even 20 minutes walking helps GI motility
  • Stool softeners (docusate/Colace) are safe and non-habit-forming for ongoing use
  • If fiber and fluids don't help, osmotic laxatives (polyethylene glycol/MiraLax) are well-tolerated
  • Avoid iron supplements and calcium-based antacids — both worsen constipation

GLP-1 medications slow gastric emptying — this is the mechanism behind weight loss, but it also slows bowel transit. Constipation affects about 24% of semaglutide users and is often underreported.
