GLP-1 Side Effects
Diarrhea
On Ozempic, Wegovy, Mounjaro & Zepbound
When it happens
Can occur any time in the dose cycle, but often appears in the first 2–3 days post-injection
Typical peak window
Day 1–4 after injection
How long it lasts
Often improves within the first 8–12 weeks as gut motility adjusts. May flare briefly with each dose increase.
What helps
- •Stay well-hydrated — diarrhea is the fastest path to dehydration on GLP-1s
- •BRAT diet (bananas, rice, applesauce, toast) on bad days
- •Avoid high-fiber foods, dairy, and artificial sweeteners during flare-ups
- •Over-the-counter loperamide (Imodium) is generally safe and effective for acute episodes
- •Probiotics may help some people — discuss with your provider
By medication
Mounjaro / Zepbound (tirzepatide)
Diarrhea rates may be slightly lower on tirzepatide than semaglutide, though both GIP and GLP-1 receptor activity can affect GI motility.
Dose context
Roughly 30% of GLP-1 users experience diarrhea. It tends to be milder and more transient than nausea for most people.
When to contact your provider
If diarrhea is severe, lasts more than 48 hours, or involves blood. Watch for signs of dehydration: dizziness, dark urine, rapid heartbeat.
