Side Effects/Fatigue

GLP-1 Side Effects

Fatigue

On Ozempic, Wegovy, Mounjaro & Zepbound

When it happens

Often most noticeable in the first 1–2 days post-injection as levels rise rapidly

Typical peak window

Day 13 after injection

How long it lasts

Usually improves as caloric intake stabilizes. If eating very little (common early on), fatigue is largely from caloric deficit, not the drug directly.

What helps

  • Make sure you are eating enough — undereating dramatically on GLP-1s is common and causes significant fatigue
  • Prioritize protein: aim for 0.7–1g per pound of body weight to preserve energy and muscle mass
  • Stay hydrated — even mild dehydration amplifies fatigue
  • Light exercise (walking, stretching) typically helps more than rest for GLP-1 fatigue
  • Avoid scheduling demanding work or activities on Day 1–2 post-injection if fatigue patterns predictably

Dose context

Some fatigue is expected early on. It is often a sign the drug is working — your body is adjusting to reduced caloric intake. Protein and hydration are the main levers.

When to contact your provider

Extreme fatigue that doesn't improve after 2–3 weeks at stable dose, or fatigue accompanied by shortness of breath or chest tightness.

Understand your dose cycle

Your drug level right now explains a lot about how you feel. See exactly where you are in your cycle.
